Form name
This is the name displayed in the pull-down menu when placing a form in a page. Form title
This is the title that will appear at the top of the form when displayed on the live site. Form recipient's email
Type the email address of the person in your organization whom you wish to receive the information submitted via the form. You can designate as many recipients as you like, just separate each email address with a comma. Store results in a table
Some sites are set up to store and access submitted form data for other proceedures. If your site supports this, select this option to store the submitted information in your site's database. You may use this in conjunction with sending the information to a designated individual.
If your site does not support this feature, do not select this option. URL to go to once the e-mail is successfully sent
Once the form has been successfully filled out and submitted, the browser will load a different page. Type the URL for that page in this field. You might want to create a generic "Thank you" page that all forms will display after the viewer submits their information.
To do this, first create the page in the Page editor and preview the page. Copy the URL from the address bar and paste it address into the Form URL field (you may delete the &Preview=1 from the end of the address). Form instructions
This is the text that will appear under the title. It should explain anything a visitor might need to know about the form before he or she fills it out. It is usually a good idea to remind visitors to fill in all required fields, which are marked with a red asterisk. Once you have entered some or all of this information, click Create form to begin work on the form itself. You can change or add to any of the above information at any time. Save any changes by clicking Update form below the Form instructions field. Return to top Preview the form
To preview the form, click the Preview form link at the top of the form editor page (below the Site manager navigation bar).
This opens the form in a new browser window so you can see how it will appear to the visitors. Close the preview window to make further modifications the form or to exit the Form editor. Exit the Form editor
You can exit the Form builder at any time by clicking the Exit button. If you have made any changes to the form information, remember to click Update form before exiting. Return to top
