After you click Create form, you will see tools at the bottom of the page that are used to create the form fields. You create one field at a time, with each field being either a single line or a multi-line answer. Click the following for more information about Form fields: Field content

Enter the text that describes what you want the visitor to input into this particular field. For example, if you want their first name, enter "First Name". Type
The Form editor has two choices for visitor input: single line or multiple line. Some information you want from you visitor will be in the form of a short one-line answer -- a name or email address for example.
Other information may require more than one line, such as fields for mailing addresses or comments. If you only need one line, choose single line. If the information requires more than one line, choose multiple line -- you can choose the display height of the field under the "Height" column. Return to top Required
If you select Yes, visitors won't be able to submit the form without first entering information in this field. If you select No, they may leave the field blank.
Examples of fields you might want to make required are phone numbers or email addresses when you are gathering addresses for a newsletter. Fields that don't need to be required might include fields for middle initials or extra comments. Return to top Width
This is the field display width (in pixels) of the area in which the visitor will type the information requested. Height
If you selected "multiple line," this is where you choose the height of the text box (this column does not apply for single line fields). When you have filled in the appropriate information, click the Save field button. The field is then added to your form and appears with its attributes at the bottom of the page. To edit a field, click on the corresponding pencil icon under the "Edit field" column. Make any modifications you wish, then click Update field to return to the Form editor. Click Cancel to disregard any modifications and return to the Form editor. Deleting a field
To delete a field, click on the corresponding X icon under the "Delete field" column. You will be presented with a display requiring confirmation that you want to delete the selected field.
If you want to continue with the delete action, click Delete field to return to the Form editor with the field removed (all subsequent fields will move up to fill the empty spot). If you do not want to continue with the delete action, click Cancel to return to the Form editor with no changes. Return to top
